  • The PEPE (Positive Expiratory Pressure Exerciser) provides Positive Expiratory Pressure (PEP). PEP devices are clinically acknowledged as potentially beneficial for sufferers of childhood and adult Asthma, Cystic Fibrosis, Bronchitis and other Chronic Obstructive Pulmonary Diseases, and pre and post COVID 19 conditions*.

     

    The PEPE device is designed for the individual personal user and has a mouthpiece at one end, a plastic shielding, punctured wrap at the other end, and a solid stainless-steel sphere which rests in a plastic circular cone on the inside of the device.  The ball floats up and down with the user’s forced expirations through the device.

  • The PEPE device is described as: Respiratory medical apparatus and instruments, namely, a handheld personal use medical device that a user breathes into for loosening mucus build-up from the user’s airways and for the treatment of respiratory diseases and conditions; mucus extractors; medical therapy apparatus, namely, lung physiotherapy apparatus for use in loosening and eliminating mucus build-up or phlegm from airways; lung exercise apparatus for physical therapy purposes; medical respiratory apparatus for improving the strength of a user’s inspiratory and expiratory muscles; medical apparatus for strenuously exercising the breathing muscles in order to improve their performance.

The PEPE device objective is its ability to:

  • Vibrate the airways which loosens mucus from the airway walls
  • Intermittently increase endobronchial pressure which assists to maintain the clearing of the airways while a person exhales so that mucus does not become trapped as it moves up the air canal
  • Accelerate expiratory airflow which facilitates the upward movement of mucus through the airways so that it be more easily cleared

The steel ball’s oscillatory movement is function of the air flow rate and angular orientation of the device.  When the ball’s oscillatory frequency matches the natural frequency of the patient’s chest wall and abdomen system, it will produce resonance which in turn, will enhance sputum clearance.

“The PEPE Device must be used in the sitting or supine lying position. The patient inhales deeply and holds his/her breath for two to three seconds.  Expiration should be slow and controlled through the PEPE valve, causing oscillations of the steel ball inside the cone of the device.  Patients apply repeated exhalations through the valve.  Routinely, three sets of 15 exhalations are performed over 12-20 minutes.  After each series of exhalations, patients are instructed to “huff” and cough, thereby aiding expectoration.” The PEPE device may be sterilised for repeat personal use.
